This is the official election notice identifying the Districts and names of incumbents holding current positions on the County Board.

2012 BALLOT ACCESS CHECKLIST

This checklist will tell you the necessary forms which must be completed and filed on time by a candidate for the Office of County Board Supervisor or County Executive in order for the candidate's name to be placed on the ballot.

CAMPAIGN REGISTRATION STATEMENT

If a candidate does not file this statement by the deadline for filing nomination papers, the candidate's name will not be placed on the ballot.

DECLARATION OF CANDIDACY

All candidates seeking ballot status for election to any office in the State of Wisconsin must properly complete and file a Declaration of Candidacy.

NOMINATION PAPER FOR NONPARTISAN OFFICE

Candidates running for the Office of County Board Supervisor must collect between 20-100 signatures of individuals living in their district in order for the candidate's name to appear on the ballot.
